The Health Foundation of Greater Cincinnati
The Health Foundation of Greater Cincinnati is an independent not-for-profit social welfare organization dedicated to improving community health. The foundation awards grants to non-profit and governmental organizations for programs and activities that improve health in Cincinnati and 20 surrounding counties in Indiana, Kentucky, and Ohio.

Physician Practices
YourHealthMatters collects information from physician practices that have chosen to voluntarily participate in this initiative. These providers are willing to give the public access to objective information about their quality of care, and who are committed to improving it. By publicly reporting their clinical diabetes and cardiovascular disease results, they are demonstrating their commitment to providing quality to their patients and to our community. Through such excellent leadership, we have been greatly assisted in helping Greater Cincinnati become a national model for health care improvement.
