Did you know... the Greater Cincinnati region currently ranks among the highest in the nation per capita for Patient Centered Medical Homes (PCMH), a new and innovative way of providing primary care services using a team approach? One analysis showed that patients enrolled in Cincinnati medical homes had fewer visits to the Emergency Department and fewer hospital admissions over the four years from 2008-2012 than patients from practices that weren’t medical homes.
